PARENTS: PLEASE NOTIFY US IF YOUR CHILD HAS BRACES AND WHEN THEY ARE SCHEDULED TO COME OFF.

Ages 0 to 3
Mail a snapshot with the following information on the back of the photo:

  • baby's name
  • date of birth
  • eye color
  • hair color
  • sizes
  • parents' names
  • home phone number
  • work phone number
  • cell phone number

If you are submitting your child to our firm, you should print the application for a 6 month New York State Child Performer Work Permit and mail it into the New York State Department of Labor. It must be renewed every six months. All children under the age of 18 cannot work without it. The application is available here.
